For y = mx + b, we will use the x and y from the given point (-3, -1) and the slope (m) of 2.  -1 = 2(-3) + b.  -1=-6+b and b = 5.  So the equation in slope-intercept form is y = 2x + 5, I think choice b (but you have an = sign in place of the + or -).
